February 23rd - Professional Documents, Applications & $100 deposit due.

All licensed professionals must submit the following:

a. Current copy of Medical License

b. Copy of Diploma

c. Copy of Vital Stats page of Passport

March 22nd - Application, passport and full payment of $1700 team fees due.
